About The Position

DaVita is looking to expand its team and bring on a Full-time Bilingual Social Worker to join their team. This position is part of a team that values work-life balance and where personal and professional growth is a top priority. This role is 4 days a week, with 1 day in Oakland and 3 days in Alameda. The Bilingual Social Worker will be a vital member of each patient’s core care team, supporting and advocating for patients transitioning to life on dialysis. The role involves helping patients understand their rights and responsibilities, and guiding them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease. The position offers the opportunity to build meaningful relationships with patients and their families in an intimate outpatient setting, and to be part of a supportive team environment with performance-based rewards.

Requirements

Meets all state required regulations to practice in the dialysis setting.
Master's degree in Social Work (MSW) required with a specialization in clinical practice
Demonstrated knowledge of government and private insurance programs
Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days
Must Be Bilingual in Spanish

Responsibilities

Support and advocate for patients transitioning to life on dialysis.
