About The Position

First Choice Health Centers is seeking a bilingual Behavioral Health Clinician fluent in English and Spanish to provide high-quality care to patients in East Hartford and Manchester, CT. This role involves providing psychotherapy to adults and children, participating in educational activities, and contributing to service improvement within a community health center setting. Requirements

• Master’s degree from an accredited school of Social Work
• Must have an independent license in the state of CT (LMSW or LCSW)
• Bilingual – oral fluency in English & Spanish is preferred
• A desire to work with the underserved population
• Experience in providing clinical services

Responsibilities

• Evaluate and diagnose mental, emotional, developmental and behavioral health challenges in individuals, couples and families based on established clinical standards.
• Conduct patient screening using standardized tools.
• Formulate treatment plans and reviews.
• Provide mental and behavioral health treatment to patients in an integrated community healthcare clinic.
• Provide treatment, including individual, group and/or family therapy, and some substance abuse counseling when necessary.
• Coordinate with other clinical and social service providers and care coordinators.
• Document all aspects of clinical care in the electronic health record (we use eCW) within required timelines.
• Ensure patient confidentiality at all times.
