About The Position

Renalogic is dedicated to helping our clients manage the human and financial costs of chronic kidney disease. We are looking for humble, hungry, and smart individuals with a sense of humor to join our mission. Our high client retention rate indicates we are on the right path. As a Clinical Nurse Coach, you will provide remote health coaching to members at risk for or already diagnosed with chronic kidney disease. Your role will involve partnering with members to enhance their understanding and management of conditions and behaviors impacting kidney health, ultimately helping them avoid dialysis. This specific role requires working three late shifts per week, residing in the Pacific or Mountain time zone, and fluency in both English and Spanish.

Requirements

  • RN license in good standing and willingness to obtain licensure in other states.
  • A minimum of five years of RN experience.
  • Must live in PST or MST.
  • Ability to fluently speak, read, and write in both English and Spanish.
  • Ability to attend and professionally engage in video meetings.
  • Proficient technological skills, including effective and efficient use of computers, peripheral equipment, and applications/systems, including Microsoft products.
  • Autonomous self-starter comfortable with ambiguity.
  • Creative mindset and ability to appropriately challenge the status quo.
  • Superb written and oral communication skills.
  • Ability to overcome obstacles with a ‘yes if...’ approach.
  • Ability to effectively balance competing deadlines without losing focus on the bigger picture.
  • Reliable internet and power with a designated area to conduct work with minimal interruptions.
  • Must pass a background and reference check.
  • Must demonstrate legal authorization to work in the United States without sponsorship.

Nice To Haves

  • Experience developing care plans for individuals with chronic conditions and/or Chronic Kidney Disease.
  • BSN.
  • CDE or CDCES.
  • Experience working 100% remote as a nurse.
  • Ability and willingness to travel occasionally, which will include overnight stays for corporate gatherings, conferences, and health fairs.

Responsibilities

  • Establishing and maintaining contact with assigned and active members via video, phone, text, and email in both English and Spanish.
  • Obtaining and utilizing clinical information to develop an individualized member care plan and clinician-centered care plan per NCQA guidelines.
  • Assessing members health status and care coordination needs.
  • Collaborating with members’ health care providers as needed.
  • Utilizing motivational interviewing techniques, individualized care plans, and the Triple Aim framework to empower active members to reach their health goals.
  • Understanding and explaining to new or inactive members the benefits of our Program as it relates to those with chronic kidney disease and those at risk.
  • Appropriately documenting, requesting, and sending member information in accordance with HIPAA compliance.
  • Meeting individual performance metrics to support member engagement and retention.
