Position Summary This Case Management Analyst position is with Aetna’s Long-Term Services and Supports (LTSS) team and is a field-based position. Candidates must be located in Miami-Dade County, specifically in zip codes 33142 or 33147. This role will require 50-75% travel for face to face visits with members in Miami-Dade County. Schedule is Monday through Friday, 8:00am - 5:00pm, Standard Business Hours. We are seeking self-motivated, energetic, detail oriented, highly organized, tech-savvy Case Management Analyst to join our Case Management team. Case Management Analyst is responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness. Case Management Analyst will effectively manage a caseload that includes supportive and medically complex members. Case Management Analyst will develop a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration. Case Management Analyst will determine appropriate services and supports due to member's health needs; including but not limited to prior authorizations, coordination with PCP and skilled providers, condition management information, medication review, community resources and supports.

Requirements

  • Bilingual: (English & Spanish).
  • Must reside in zip codes 33042 or 33147.
  • Willing and able to travel 50-75% of their time to meet members face to face in Miami-Dade County and surrounding areas in Florida.
  • 1+ year of experience in behavioral health or a long-term care setting.
  • Computer proficiency in Microsoft Word, Excel, and Outlook required.
