CVHS is seeking a full-time Behavioral Health Care Coordinator with a Bachelor’s degree in Social Work or related field to provide behavioral health care coordination, case management, and resource support services within an integrated primary care setting. Services will be provided to a diverse patient population with complex behavioral health and social needs. The Behavioral Health Care Coordinator will assist with outgoing referrals for higher levels of care, conduct pre-behavioral health diagnostic screening visits, address social determinants of health, and assist patients with applications for disability and other social service resources. This role will also involve networking with community agencies, maintaining regional resource relationships, and collaborating closely with behavioral health clinicians and medical providers as part of a multidisciplinary care team. While this position is primarily remote/work from home, there is an expectation to travel to assigned clinics as needed to meet with patients, network within the community, attend in-person meetings, and participate in occasional trainings. Candidates must reside in Virginia and preference will be given to applicants who are familiar with the Fredericksburg, Caroline, Westmoreland, and/or King William areas. Candidates must be comfortable documenting all services in the electronic medical record in a timely manner and working in a fast-paced healthcare environment. Experience in behavioral health, healthcare, and/or case management preferred. Spanish-speaking preferred but not required. Competitive benefits available.
