About The Position

Assists with ensuring bereavement services are provided to the families/caregivers of hospice patients in accordance with the interdisciplinary plan of care and in alignment with SCAH, mission, goals and objectives. Bereavement counseling consists of counseling given to the individual's family after the individual's death. Bereavement counseling is a required hospice service, provided for a period up to one year following the patient's death. Bereavement counseling will ensure that the bereavement services reflect the needs of the bereaved.

Requirements

Master's degree in social work or related field (equivalent experience accepted).
Valid Driver's License.
Automobile Insurance.
BLS - Basic Life Support Certification.
2 years recent relevant experience.
Understanding of psychosocial implications of illness, hospice and/or home care death and dying issues and dynamics of grief.
Knowledge of Medicare, Accrediting bodies, State and all other regulations applicable to Bereavement programs.
Knowledge of community resources available to bereaved persons.
Excellent computer skills.
Ability to discern sensitive issues and respond appropriately.
Sensitivity to human needs and situations encountered in hospice and bereavement care.
Detail-oriented and creative.
Excellent organizational skills, interpersonal skills, problem-solving abilities, ability to prioritize work.
Ability to work effectively with a minimum of supervision.
Ability to work under time pressure and changing priorities and deal with potentially conflicting demands.
Demonstrate professionalism when interacting with clients and co-workers.
Ability to handle sensitive issues, conflict with or among others, respectively direct and reinforce staff efforts; effectively plan and organize and prioritize work, and to effectively solve unique problems as they arise or identify when to consult supervisor.
Ability to deal with a challenging work environment from time demands and conflicting priorities.
Maintain a positive and professional image and demonstrate self-directed learning to meet professional development and/or license requirements.
Ability to meet core competencies for position and area of specialty at orientation and annually thereafter.
