Pathways Hospice has an immediate opening for a Bereavement Counselor. This role provides grief counseling and support to bereaved children, adults, and families, as well as individuals with a serious illness. The Bereavement Counselor delivers continuity of care between direct-care and bereavement components of hospice services, facilitates bereavement support groups, and engages in community outreach programs. The position requires the ability to provide emotional, psychosocial, and spiritual support before and after a patient's death to assist with grief, loss, and adjustment. Responsibilities include conducting bereavement assessments, developing and maintaining bereavement plans of care, providing crisis intervention, and coordinating special grief and support services. The role also involves delivering grief counseling and education to the community through support groups and presentations.
