About The Position

The Bereavement Coordinator is responsible for planning, implementing, and evaluating bereavement services for patients' families and community. This role includes providing emotional support, developing individualized care plans, coordinating community resources, and offering grief support. The coordinator ensures that families receive compassionate support after the loss of a loved one, in alignment with hospice care values.

Requirements

Bachelor's Degree in social work, counseling or related area required.
State licensure as appropriate in Kentucky and Ohio.
At least one year experience in a health care setting.
Knowledge of current trends in bereavement care and end-of-life experience.
Experience in the area of grief and loss support and group activities.
Ability to communicate professionally with clients, team members, providers, and healthcare professionals.
Personal qualities of empathy, responsibility, diplomacy, dependability and integrity.
Effective communication skills with team members, clients, and family members.
Ability to work independent of direct on-site supervision.
Leadership and teaching ability.
Must carry state minimum state minimum in automobile liability insurance.

Nice To Haves
