The Bereavement Coordinator is responsible to plan, implement, and maintain a bereavement program to meet the needs of families/caregivers from VitalCaring for up to one (1) year following the death of the individual hospice patient in concert with our values: trustworthy, capable, compassionate, proactive, and called. Requirements

Bachelor’s degree or equivalent theological degree from college or university. Experience with families/caregivers and patients through death/dying issues – OR – Master's degree in Social Work with qualifications of experienced clinical grief counseling.
Documented evidence of appropriate training and experience in the care of the bereaved received under the supervision of a qualified professional.
Knowledge and commitment to hospice philosophy.
Understanding of grief response and experience with counseling individuals and/or in groups regarding loss.
If making home visits, you must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

Responsibilities

Implements bereavement counseling in a manner consistent with standards of practice.
Assesses hospice family to identify grief counseling needs as evidenced by documentation, and community resource referrals.
Provides bereavement information and referral services to the bereaved, as needed, in accordance with the patient’s Plan of Care.
Provides bereavement support to hospice staff as needed.
Documents bereavement services provided and progress of bereaved on a clinical progress note that is incorporated in the electronic medical record.
Attends IDG meetings as needed to promote coordination of bereavement care.
Meets mandatory continuing education requirements of the Hospice.
Demonstrates commitment, professional growth, and competency by maintaining a working knowledge of public and private eligibility standards.
Promotes Hospice philosophy and administrative policies to ensure quality of care.
Provides continued bereavement support to hospice families as evidenced by documentation.
Uses effective interpersonal relations and communication skills.
