About The Position

The Bereavement Coordinator is responsible to plan, implement, and maintain a bereavement program to meet the needs of families/caregivers from VitalCaring for up to one (1) year following the death of the individual hospice patient in concert with our values: trustworthy, capable, compassionate, proactive, and called. Requirements

  • Bachelor’s degree or equivalent theological degree from college or university. Experience with families/caregivers and patients through death/dying issues – OR – Master's degree in Social Work with qualifications of experienced clinical grief counseling.
  • Documented evidence of appropriate training and experience in the care of the bereaved received under the supervision of a qualified professional.
  • Knowledge and commitment to hospice philosophy.
  • Understanding of grief response and experience with counseling individuals and/or in groups regarding loss.
  • If making home visits, you must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

Responsibilities

  • Implements bereavement counseling in a manner consistent with standards of practice.
  • Assesses hospice family to identify grief counseling needs as evidenced by documentation, and community resource referrals.
  • Provides bereavement information and referral services to the bereaved, as needed, in accordance with the patient’s Plan of Care.
  • Provides bereavement support to hospice staff as needed.
  • Documents bereavement services provided and progress of bereaved on a clinical progress note that is incorporated in the electronic medical record.
  • Attends IDG meetings as needed to promote coordination of bereavement care.
  • Meets mandatory continuing education requirements of the Hospice.
  • Demonstrates commitment, professional growth, and competency by maintaining a working knowledge of public and private eligibility standards.
  • Promotes Hospice philosophy and administrative policies to ensure quality of care.
  • Provides continued bereavement support to hospice families as evidenced by documentation.
  • Uses effective interpersonal relations and communication skills.
