About The Position

The Bereavement Coordinator is responsible for planning, implementing, and maintaining a bereavement program to meet the needs of families/caregivers for up to one (1) year following the death of the patient.

Requirements

  • Strong communication skills and interpersonal skills.
  • Valid driver’s license and proof of insurance are required.
  • Possess high integrity, including the maintenance of confidential information.
  • Minimum high school diploma required.
  • Upon hire, participate in online Bereavement Coordinator training.
  • Must be sensitive to the human needs and situations encountered in the provision of hospice and bereavement care.

Nice To Haves

  • Associate’s degree or higher preferred.

Responsibilities

  • Coordinates the assessment and delivery of grief support needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers.
  • Provides bereavement information and referral services to callers from the community. Expands and maintains community resource list, as needed.
  • Functions independently, but seeks and accepts guidance from others.
  • Participates in hospice activities such as in-service education and hospice personnel meetings.
  • Organizes annual memorial services for patients who were under our care.
  • Oversees the mailings, phone calls, and visits to family/caregivers in the program.
  • Functions as part of the IDT team.
  • Attend continuing education on dying and the grieving process on an annual basis.
