About The Position

This position provides Community Health Initiatives services as part of Mazzoni Center's ending the HIV epidemic programming, including but not limited to Linkage to Care services, Navigation services, and other programming related to ending the epidemic. Communication: Good oral and written communication skills are required for this position. The person in this position must be able to communicate effectively with the clients, staff, and others; and contribute to a positive environment.

Requirements

  • Commitment to the mission of Mazzoni Center
  • Bachelor’s Degree required, Bachelor’s Degree in Social Services (Psychology, Social Work, Nursing, Public Health or another related field) preferred
  • Knowledge of HIV/AIDS prevention, treatment, and community resources
  • Cultural competency in working with LGBTQ+ and diverse populations
  • Excellent organizational skills and follow through
  • Good written and oral communication skills
  • Flexible with great time management skills
  • Background checks are required, including Criminal Background Clearance, Child Abuse Clearance, and FBI Clearance
  • The ability to stand, sit and walk for extended periods of time
  • The ability to lift objects greater than 35 pounds
  • The ability to communicate using written and spoken words

Responsibilities

  • Maintain a caseload and service unit production as determined by funding requirements and/or in accordance with intervention/programmatic expectations, policies, and procedures
  • Provide program service deliverables and meet program goals
  • Assist with program promotion and participant recruitment through various forms of community outreach
  • Provide interval client assessments to identify new or emerging needs as deemed necessary by the program, the needs of the individual and/or by the instruction of the supervisor
  • Maintain minimum contact with clients based on program and client need
  • Identify appropriate resources and/or services to address client needs and link clients to those resources and/or services
  • Links clients to medical care and other services and resources
  • Coordinate and monitor ongoing services
  • Assist client with identifying, building, and maintaining external support systems
  • Provide education and resources around sexual health, HIV, STIs, case management, mental health, treatment services, education, housing, employment, etc.
  • Supports clients in understanding how to access health care, services, programs, bio-medical prevention strategies, etc.
  • Perform HIV and other STI prevention counseling, phlebotomy, blood and specimen processing, and HIV and other STI screening as necessary
  • Maintain communication and coordinate with internal and external providers to ensure service linkages
  • Confirm client's completion of referrals
  • Supports other team members as needed, which can include, but is not limited to working on groups, workshops, training, events, and performing reception duties
  • Provide services, outreach, and support to accommodate program and client needs as necessary (including, but not limited to special events related to programming)
  • Maintains excellent communication with all internal and external team members
  • Maintain accurate written documentation of all client contacts and progress in Careware (using D.A.P. format), AthenaNet, Evaluation Web, internal spreadsheets and/or other data programs. Service units and case notes should be input within one (1) business day of contact
  • Maintain accurate and orderly client files with hard copy of client-related forms, applications, correspondence, etc.
  • Always protect client information and records in compliance with HIPAA, agency Confidentiality Policy and Pennsylvania Act 148
  • Adhere to Mazzoni Center and OSHA safety guidelines, i.e., infection control, immunization policies, exposure control (blood borne pathogens and chemical hazards)
  • Participate in weekly supervision
  • Participate in team meetings, department meetings, agency meetings, staff in-services, and external trainings as required
  • Assist with other departmental needs
  • Perform other duties as assigned by Supervisor or Department Director
