About The Position

Works with individuals, groups, or communities to facilitate positive and healthy behaviors. Interacts with adolescents who have congenital heart disease using a theory-based lifestyle intervention to increase physical activity.

Requirements

  • Bachelor’s degree in a relevant area, including exercise physiology, nursing, physical therapy, psychology, community wellness, social work, or another health-related field, required.
  • Pursuing an advanced degree in a relevant area as mentioned above.
  • An interest in working with adolescents, required.

Responsibilities

  • Conducts assessments to identify behaviors and develop intervention plans.
  • Monitors and evaluates the effectiveness of interventions and modifies plans as needed.
  • Collaborates with other professionals, such as psychologists and social workers, to provide comprehensive care.
  • Provides training and support to caregivers and other professionals to ensure consistency in intervention strategies.
  • Maintains accurate and up-to-date records of client progress and communicates with relevant parties as needed.
  • Maintains accurate and timely documentation of all interventions and progress.
