Behavioral Health Triage Clinicians coordinate care for patients seeking substance abuse treatment and services. They are typically the first staff members patients meet during the assessment process. Clinicians triage patients for risk and acuity, document triage in the electronic medical record (EMR), and take vital signs in coordination with the Access RN. They gather demographic and registration information, verify insurance, collect payments as needed, gather referral information, document social determinants of health (SDOH), complete initial screening for suicidal or homicidal ideation (SI/HI), and complete EMR documentation. They also obtain signatures for required documents, including releases of information (ROIs) and financial waivers, and hand off a case overview to a licensed clinical professional or Access Therapist. The role includes monitoring patients and supporting safety in the Access department, as well as assisting Access Therapists with admissions as needed. Clinicians also respond to inquiries about services, gather information to determine whether callers should be scheduled for an assessment or provided referrals, document caller demographics, insurance verification, and reasons for inquiry, and direct questions and calls to the appropriate staff.
