About The Position

The Southeast Permanente Medical Group (TSPMG) is one of Georgia’s largest independent, physician-owned, multi-specialty medical groups. We invite applications for a licensed Behavioral Health Transitional Case Manager LCSW to join our busy practice located in metropolitan Atlanta.

Requirements

  • Licensed Clinical Social Worker
  • 2 - 5 years inpatient psychiatric care
  • 2 - 5 years discharge planning experience and/or transitional case management or complex case management experience working with behavioral health patients

Responsibilities

  • Provides direct behavioral health services within scope of practice, including post-hospitalization assessments, individual therapy, group therapy, and other clinically appropriate interventions
  • Maintains a rolling caseload of recently discharged members and conducts timely outreach, including multiple contact attempts as appropriate, to engage members and caregivers following discharge
  • Completes comprehensive assessments of members' psychosocial, behavioral health, environmental, financial, and functional needs; evaluates readiness for self-management and identifies factors that may affect treatment engagement outcomes
  • Utilizes clinical judgement to identify members at risk for relapse, deterioration, avoidable emergency department utilization, readmission, or disengagement from treatment, and escalates concerns to the appropriate care team members
