This position is responsible for accepting referrals regarding patients’ behavioral health needs, responding in a timely manner, and providing and documenting referrals as needed to appropriate providers/resources. The role involves providing comprehensive assessment, diagnosis, consultation, and interventions for behavioral health, substance use disorder, and social determinants of health needs. The therapist will also provide counseling services on an individual, couples, family, and/or group basis. Additionally, the position assists the care team in developing care management processes, consults and collaborates with patients and the care team regarding recommended behavioral health interventions, and assists in detecting “at risk” patients to develop prevention plans. The therapist is expected to meet quality and utilization standards, maintain accurate documentation, monitor behavioral health needs at assigned sites, and participate in relevant meetings and huddles. A positive and professional attitude is expected, along with the ability to interact well with colleagues and leadership.
