In this role you will, support our patient transitions from inpatient care by coordinating recommended aftercare services based on clinical assessments provided by primary therapists.

Requirements

• Knowledge of mental health and/or substance abuse treatment standards.
• Bachelor's degree required.

Nice To Haves

• Emergency psychiatric and/or acute psychiatric experience preferred.
• Master's degree or above from an accredited college or university with a major in psychology, social work, counseling, nursing or other related field preferred.

Responsibilities

• Coordinate aftercare services based on clinician-recommended levels of care (LOC); does not determine clinical LOC.
• Collaborate with referral sources to ensure timely, appropriate aftercare placements.
• Arrange housing and residential referrals, including HWH and RH as indicated.
• Communicate and coordinate with BHT and Business Development teams regarding aftercare needs.
• Facilitate a monthly community education session on the benefits of engaging in recommended aftercare (PHP, IOP, OP, supportive housing).
• Maintain accurate documentation related to aftercare coordination.
