Requirements

  • Master's Degree in Social Work with a minimum of two years' experience as a therapist and a current license to practice in Michigan as a Licensed Clinical Social Worker
  • State of Michigan Clinical Social Worker License
  • BLS required within 6 months of hire

Responsibilities

  • Promote and actively engage in behavioral health integration within primary care
  • Conducts therapy sessions with individuals or groups
  • Formulate, develop, and implement a comprehensive psychosocial treatment plan utilizing appropriate clinical treatments and interventions
  • Provide quality psychosocial assessments of patients/clients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment and/or life situation
  • Ensure adequate programmatic data is collected, analyzed, and presented as necessary.
  • Assist in the development and distribution of Behavioral Health Program and Substance Abuse Program procedures/policies
  • Consultation with PCP's to enhance understanding of the patients/clients, provide decision support for treatment planning and assist in the implementation and monitoring of biopsychosocial treatment plan
  • Documents all patient/client related interactions and completes progress notes and discharge plans in a timely manner
  • Maintain confidentiality in compliance with HIPPA and RHC regulations
  • Participates in Collaborative Care Program
