Here, you are not just joining a workplace, you are becoming part of a mission that truly matters. We are currently seeking a Behavioral Health Therapist (LCSW/LSW, LMHC/LMHCA...) to join our team. This position offers the chance to serve an established panel of patients in a medically underserved community while working alongside in-house wraparound support, including Outreach Specialists, Care Managers, and Patient Care Coordinators.

Requirements

  • Master’s Degree in Social Work, Counseling, or Marriage & Family Therapy
  • Active Indiana licensure as LCSW/LSW, LMFT/LMFTA or LMHC/LMHCA (required)
  • Relevant experience in healthcare, mental health, or community-based services
  • Strong knowledge of disease processes, human behavior, and principles of growth and development
  • Ability to provide effective therapeutic counseling and crisis intervention
  • Excellent interpersonal and communication skills; ability to collaborate in a team-based setting

Nice To Haves

  • CPI (Non-Violent Crisis Intervention) certification preferred

Responsibilities

  • Conduct comprehensive psychosocial assessments and develop individualized treatment plans
  • Provide short-term therapeutic counseling and crisis intervention to patients and families
  • Collaborate with physicians, nurse practitioners, and the broader care team to ensure integrated care
  • Coordinate services and support smooth transitions across levels of care
  • Maintain accurate and timely clinical documentation in the Electronic Health Record (EHR)
  • Apply knowledge of protective service mandates for both adults and children
  • Utilize de-escalation and crisis intervention techniques when needed
  • Build and maintain strong relationships with community resources, medical teams, and protective services
  • Participate in team-based care meetings and continuous improvement initiatives
