About The Position

Community Healthcare Network (CHN) is a not-for-profit organization providing more than 65,000 New Yorkers with primary and behavioral healthcare, dental, nutrition, wellness, and needed support services. Community Healthcare Network is seeking a Full-Time Behavioral Health Therapist who will work as an integral part of the Behavioral Health treatment team providing clinical treatment services under the direction of the Behavioral Health Director and treating Psychiatrist.

Requirements

  • M.S.W Degree
  • Licensed Master Social Worker
  • Licensed Clinical Social Worker

Responsibilities

  • Provides psycho-social assessment and screening services to individuals interested in becoming participants in the mental health program.
  • Facilitates related services for health center patients as appropriate with respect to their confidentiality and privacy.
  • Provides treatment services to a caseload of 30-50 clients, the caseload number being based on the following factors: show rate, modalities provided (i.e. individual therapy only, group and individual therapy) and maintaining weekly utilization/billing targets.
  • Evaluates assessments and makes diagnoses; develops and implements treatment plans according to OMH standards.
  • Provides psychotherapy and psycho-education to a variety of clients, utilizing individual, family and group models.
  • Participates in regular clinical supervision to discuss cases.
  • Involves clients in their own treatment planning as well as their own education about their mental illness(es)
  • Develops and maintains clinical records, in accordance to OMH standards.
  • Ensures that patients receive coordinated and related services.
  • Provides cases intervention, which could include home visits, escorting clients to emergency room, assisting in the development of plan for clients at risk for suicide/homicide.
