About The Position

This role involves providing comprehensive support and case management for individuals receiving behavioral health services. The technician will work as part of an interdisciplinary treatment team, building therapeutic relationships, coordinating services, and implementing treatment plans. Responsibilities include direct client interaction, group facilitation, and assisting clients in obtaining daily living necessities and support services.

Requirements

  • Ability to work independently and collaboratively as part of the interdisciplinary treatment team.
  • Attend and share appropriate information regarding clients in the daily morning meeting.
  • Advocate for clients’ rights and preferences.
  • Build therapeutic relationships with individuals served.
  • Interact professionally and appropriately with individuals, families, and other community agencies.
  • Provide service coordination (case management).
  • Coordinate and monitor the activities of the individual treatment team.
  • Develop, write, implement, evaluate, and revise overall treatment goals in collaboration with the client and the individualized treatment team.
  • Make immediate changes in treatment plans as clients’ needs change.
  • Educate and support clients and their families.
  • Provide one to one therapeutic interventions with the individual and family.
  • Facilitate group therapy, psychotherapy, symptom management and other appropriate therapeutic interventions.
  • Provide practical help and side by side support, mentoring, advocacy, coordination, problem solving, direct assistance and supervision to help clients obtain the necessities of daily living.

Responsibilities

  • Ability to work independently and collaboratively as part of the interdisciplinary treatment team, including attending and sharing appropriate information regarding clients in the daily morning meeting.
  • Advocating for clients’ rights and preferences with the team, their families, the community, etc.
  • Builds a therapeutic relationship with individuals served and effectively interacts in a professional and appropriate manner with individuals, families, and other community agencies.
  • Provide service coordination (case management) for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team.
  • Assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals in collaboration with the client and the individualized treatment team, ensuring immediate changes are made in the treatment plans as clients’ needs change.
  • Educate and support clients and their families.
  • Provides effective one to one therapeutic interventions with the individual and family.
  • Facilitates group therapy, psychotherapy, symptom management and other appropriate therapeutic interventions.
  • Provide practical help and side by side support, mentoring, advocacy, coordination, problem solving, direct assistance and supervision to help clients obtain the necessities of daily living including medical and dental healthcare; legal and advocacy services; financial supports such as entitlements (SSI, SSDI); housing subsidies (HUD, Section 8, 811); money management training: and transportation.
