About The Position
Greater Seacoast Health is seeking a compassionate and dedicated Behavioral Health Specialist to join the founding clinical team at their brand-new Wakefield Primary Care practice. As an integrated member of the healthcare team, the specialist will work alongside primary care providers, nurses, care coordinators, and other behavioral health professionals to improve the lives of individuals and families in the community. This role is ideal for someone passionate about helping people navigate mental health and substance use challenges through collaborative, patient-centered care.
Requirements
- Master's degree in Social Work, Counseling, Psychology, or another behavioral health-related field.
- Current New Hampshire licensure as an LICSW, LCMHC, MLADC, or equivalent behavioral health license, OR be fully eligible to obtain New Hampshire licensure within ninety (90) days of hire.
- Current American Heart Association (AHA) Basic Life Support (BLS) Certification (or ability to obtain upon hire).
Nice To Haves
- Experience providing outpatient behavioral health services.
- Experience treating individuals with mental health and substance use disorders.
- Community Health or Federally Qualified Health Center (FQHC) experience.
- Integrated primary care experience.
- Electronic Health Record (EHR) proficiency.
Responsibilities
- Conduct comprehensive behavioral health assessments.
- Provide evidence-based individual and group counseling.
- Deliver crisis intervention and immediate behavioral health support.
- Collaborate with primary care providers to develop integrated treatment plans.
- Support patients experiencing mental health and substance use challenges.
- Coordinate referrals and ensure continuity of care.
- Maintain accurate and timely documentation within the electronic health record.
- Participate in interdisciplinary care team meetings and quality improvement initiatives.
