About The Position

The Behavioral Health Specialist provides coordination, counseling, education, and outreach services to patients and their families/guardians. This role supports the delivery of care coordination/case management services by removing barriers within the realm of social work and enlisting community resources. Behavioral Health Specialists work in support of primary care providers and patients by providing quality clinical mental health consultation as requested. The role is an integral part of the primary care team, providing direct services. Primary responsibilities include assessing social service needs, identifying and addressing social barriers to optimal health care status (e.g., housing, transportation, utilities, food), assisting providers in accessing medically necessary specialty care, and ensuring members access appropriate preventive services.

Requirements

  • A master’s degree in social work from an accredited program or related disciplines appropriate to the position
  • Possess a current, valid and unrestricted license to practice in the state of Tennessee- LCSW
  • Maintains LCSW license during term of employment

Responsibilities

  • Completes psychosocial assessments
  • Identifies care needs and goals based upon assessment findings, analysis of claims data, provider input, and patient/parent/caregiver expressed goals
  • Provides quality clinical consultation to providers and staff regarding mental and behavioral health issues connected to medical conditions
  • Evaluates patients in crisis situations to determine appropriate intervention and facilitates external referral when necessary
  • Employs empirically-supported interventions to treat common mental health conditions and psychosocial components of chronic medical illness
  • Provides follow-up appointments to deliver psychotherapeutic care related to the mind/body connection with medical problem presentations
  • Promotes a smooth interface between medicine and specialized mental/behavioral health care
  • Collaborates and co-manages patient's behavioral health care with feedback to referring providers
  • Documents clinical behavioral health aspects of patient interventions and tracks referrals to specialty behavioral health care systems
  • Completes and submits encounter forms for billing purposes in a timely manner
  • Attends medical and behavioral health department staffing
  • Facilitates didactic sessions and discussions with presentations for Project Echo, PCMH, MIPS, and UPL
  • Convenes and facilitates meetings to ensure communication and coordination between patient, parent/caregivers, and key healthcare and support service providers
  • Consults with Quality Engagement Advisors concerning scheduling appointments, linking patients and families to community-based organizations and services, and follow-up/support for patients and families
  • Maintains and ensures the confidentiality of all Personal Health Information (PHI) according to HIPAA requirements
  • Participates in relevant continuing education offerings
  • Participates in all program-specific training programs
  • Adheres to the policies and procedures of University Clinical Health
  • Maintains strict confidentiality of client, company, and personnel information
  • Performs other work-related duties as assigned
