About The Position

The P rogram of A ll-Inclusive C are for the E lderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community. Requirements

  • Licensed (LLMSW or LMSW) Master’s degree in Social Work (MSW) or a Licensed Professional Counselor (LPC) license in the State of Michigan.
  • A minimum of 2 years of behavioral health experience, with at least 1 year working with frail, elderly, or long-term care populations.
  • Excellent communication and interpersonal skills, with the ability to work independently and collaboratively.
  • Strong organizational skills to manage a busy work environment with frequent interruptions.
  • Must be fully vaccinated or willing to become vaccinated against: Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B
  • Must meet CMS competency standards prior to working independently
  • Comfortable working in varied home environments and all weather conditions

Responsibilities

  • Comprehensive Evaluations: Conduct thorough initial evaluations and develop personalized care plans that address behavioral health and substance use disorders for our participants.
  • Team Collaboration: Actively participate in regular IDT meetings, contributing to clinical discussions and sharing your knowledge to enhance team performance.
  • Participant Care Plans: Collaborate with the IDT to create and implement care plans, set meaningful goals, and track participant progress.
  • Therapeutic Services: Provide individual and group psychotherapy, leveraging your behavioral health expertise for impactful case consultations.
  • Crisis Management: Facilitate referrals to our Behavioral Health Nurse Practitioner and coordinate care with external services while providing necessary crisis intervention.
  • Education and Training: Deliver specialized training to your team on behavioral health topics, substance use disorders, and effective behavior management strategies.
  • Documentation and Compliance: Maintain accurate records of interactions and participant progress in accordance with policies, and engage in quality improvement activities.
