About The Position

This role provides therapeutic care to participants in the Huron Valley PACE program. Under the supervision of the Behavioral Health Manager and Medical Director (or designee), the specialist delivers talk therapy through individual and group sessions. This role involves independently assessing participants' behavioral health needs, coordinating care, and implementing interventions. The specialist works closely with participants and their families/caregivers, ensuring comprehensive behavioral health services to enhance overall well-being, while collaborating with the interdisciplinary healthcare team.

Requirements

  • Master's Degree from an accredited School of Social Work or equivalent (LPC or LLPC).
  • Minimum 2 years of therapy experience.
  • LPC in Michigan, or LLPC under LPC supervision.
  • LMSW in Michigan, or LLMSW under LMSW supervision.
  • Valid Michigan Driver's License and vehicle.
  • CPR certification or willingness to obtain.
  • Knowledge of behavior principles, therapy techniques (preferably CBT and DBT), substance abuse assessment and treatment, and short-term interventions.
  • Experience or willingness to be trained in substance abuse assessment and treatment.
  • Skills in providing therapy to geriatric participants, with strong analytical and problem-solving abilities.
  • Ability to conduct home visits and provide care in various settings (nursing homes, assisted living, adult foster care).

Responsibilities

  • Provide therapeutic interventions to participants, either at HV PACE or in their residences.
  • Actively participate as a member of the Interdisciplinary Team (IDT), attend staff meetings and contribute to participant care planning.
  • Provide interventions to participants with dementia and behavioral challenges by conducting DICE Case Studies.
  • Develop and implement individualized treatment plans, which may include individual and/or group psychotherapy, consultation with Primary Care Providers and other relevant disciplines, crisis intervention, and referrals for psychiatric services.
  • Evaluate participant outcomes and progress toward the goals and objectives of the care plan and communicate this information to the IDT.
  • Complete accurate and timely documentation in electronic medical records.
  • Foster positive relationships among staff, promoting team building and cohesion.
  • Advise staff on the impact of social, environmental, emotional, and cultural factors on health and medical care, and inform them of available social services in the community.
  • Document changes in participants' conditions appropriately and communicate these updates with the IDT and family members.
  • Understand, comply with, and promote the Participant Bill of Rights, while working to ensure high levels of participant satisfaction.
