Behavioral Health Specialist – Full Time Location: Hamtramck, MI Schedule: 5 Days/Week | Hybrid (In-Clinic & Telehealth) Patient Volume: 1 Patient Per Hour Make a Lasting Impact with EPIC Health At EPIC Health, we are redefining behavioral healthcare through our fully integrated MindPATH model—embedding behavioral health directly within primary care. Requirements

  • Master's degree in Social Work, Counseling, or a related field.
  • Active LPC or LMSW license.
  • Experience conducting assessments, treatment planning, therapy, and crisis intervention.

Nice To Haves

  • Three or more years of outpatient behavioral health experience.
  • Comfortable working in a collaborative, integrated care environment.
  • Passion for serving vulnerable and underserved populations.
  • Strong communication and interdisciplinary teamwork skills.

Responsibilities

  • Conduct comprehensive mental health assessments.
  • Develop individualized treatment plans.
  • Provide outpatient therapy for adults and/or children.
  • Deliver crisis intervention and behavioral stabilization when appropriate.
  • Maintain a structured caseload of one patient per hour to ensure high-quality care.
  • Partner closely with Primary Care Providers within our MindPATH model.
  • Collaborate with care managers and interdisciplinary teams.
  • Connect patients with appropriate behavioral health and community resources.
  • Complete accurate clinical documentation, treatment plans, and progress notes.
  • Monitor patient outcomes and adjust treatment plans as needed.
  • Prepare discharge plans and case summaries.
