About The Position
Behavioral Health Specialist – Full Time Location: Hamtramck, MI Schedule: 5 Days/Week | Hybrid (In-Clinic & Telehealth) Patient Volume: 1 Patient Per Hour Make a Lasting Impact with EPIC Health At EPIC Health, we are redefining behavioral healthcare through our fully integrated MindPATH model—embedding behavioral health directly within primary care. Requirements
- Master's degree in Social Work, Counseling, or a related field.
- Active LPC or LMSW license.
- Experience conducting assessments, treatment planning, therapy, and crisis intervention.

- Three or more years of outpatient behavioral health experience.
- Comfortable working in a collaborative, integrated care environment.

- Strong communication and interdisciplinary teamwork skills.
Responsibilities
- Conduct comprehensive mental health assessments.
- Develop individualized treatment plans.
- Provide outpatient therapy for adults and/or children.
- Deliver crisis intervention and behavioral stabilization when appropriate.
- Maintain a structured caseload of one patient per hour to ensure high-quality care.
- Partner closely with Primary Care Providers within our MindPATH model.
- Collaborate with care managers and interdisciplinary teams.
- Connect patients with appropriate behavioral health and community resources.
- Complete accurate clinical documentation, treatment plans, and progress notes.
- Monitor patient outcomes and adjust treatment plans as needed.
- Prepare discharge plans and case summaries.
