The primary purpose of this position is to assess and support referred residents who demonstrate behaviors that can result in adverse outcomes for themselves or others and act as a resource person, providing behavioral strategies to their care team. The identified target population includes residents residing in Hebrew Home (Long Term care/nursing home). Ensure residents referred as the result of behavioral health concerns receive appropriate treatment and services to correct the assessed problem or attain the highest practicable mental and psychosocial well-being. Ensure residents referred due to behavioral health concerns who display or have been diagnosed with dementia, receive the appropriate behavioral health services to attain or maintain his or her highest practicable physical, mental, and psychosocial well-being. Support Hebrew Home in its provision of medically-related social services to attain or maintain the highest practicable physical, mental and psychosocial well-being of each resident. Create an environment of non-pharmacological behavioral strategies to address behavioral health issues related to residents/patients who have been referred. Collaborate with the Care Team in identifying and implementing behavioral modification/intervention strategies with a focus on reducing use of psychotropic medications.
