About The Position

We are an FQHC community health center dedicated to embodying the values central to American Indian cultures. The SAPTA Project Specialist is an integral member of our PCMH-centered multi-disciplinary care team. Responsibilities of the SAPTA Project Specialist also include understanding, planning, coordinating, and implementing the goals, objectives, and service deliveries of the Substance Use Prevention, Treatment, and Recovery Services (SAPTA) grant relating particularly in relation to the program’s external affairs and outreach efforts. to the program’s external affairs. It is the responsibility of the SAPTA Project Specialist to identify local substance use prevention concerns and use data to identify prevention priorities in the community, develop, and implement strategies to prevent the misuse of substances and promote mental health amongst youth and adults.

Requirements

  • 1 year of experience in public health or health services, social work, or a related field focusing on prevention of substance abuse.
  • Knowledge of available community-based social support services in the area.
  • Strong knowledge working with data and report writing.
  • Public speaking experience
  • Bachelor’s Degree in Communication, Public Relations, Health Sciences/Services, Social Work, or Public Health from a four-year college or university.
  • Able to communicate professionally in person and writing.
  • Has a clean driving record and insurance as required by the state.
  • Valid driver’s license and reliable transportation.
  • CPR/ BLS certification: Maintain a current Basic Life Support (BLS) certification issued by the American Heart Association (AHA), the American Red Cross, or an equivalent organization. Certification must include an in-person, hands-on skills assessment. Online-only certifications are not accepted.
  • Annual background checks: Consent to annual background checks as a condition of continued employment, to ensure compliance with organizational standards and eligibility requirements.
  • For-Cause Drug Screening: Comply with drug screening requirements when initiated by the organization for cause, to support a safe, compliant, and drug-free workplace (including marijuana).
  • Ongoing Compliance Requirements: Maintain up-to-date compliance with all required annual renewals, including professional licenses, certifications, physical examinations, TB testing, and mandatory regulatory trainings as assigned by the San Diego Community Health Center (SDCHC).

Responsibilities

  • Become familiar with goals, objectives, and service delivery as described in the SAPTA grant
  • Coordinate with the Cedar Branch community members to identify and review available evidence-based resources to adapt to local community needs assessment.
  • Plan and facilitate Youth Advisory Board meetings, ensuring the alignment with the grant.
  • Develop communication tools introducing the SAPTA initiative and soliciting community collaboration for its implementation
  • Collaborate with other Behavioral Health (BH) providers and staff to coordinate initiative implementation activities for seamless service delivery of community services available through the department
  • Develop a community calendar that includes locations and dates for community presentations and community survey implementation and outreach within the community.
  • Build partnerships with schools, businesses, community organizations, and law enforcement.
  • Create and implement an outreach plan by recognizing the target population.
  • Provide educational presentations to show the community SAPTA’s benefits and activities.
  • Engage leaders, community members, and other agencies to plan outreach events or programs
  • Monitor program effectiveness, evaluate data, and report outcomes to the clinic leadership and IHS.
  • Work collaboratively with SDCHC) Grant Writers, BH Case Manager, and data team to ensure monthly, quarterly, and yearly reports are completed and submitted on time.
  • Seeks ways to improve job efficiency and makes appropriate suggestions following the proper chain of command.
  • Adheres to organizational and departmental policies and procedures.
  • Attends educational offerings to keep abreast of change, comply with licensing requirements, and assist in the growth and development of associates by sharing knowledge with others.
