This role involves providing personalized, one-on-one behavioral health nursing care to patients in their homes. The nurse will have the autonomy to manage their caseload while working closely with a collaborative clinical team dedicated to helping patients remain safe, stable, and independent in their communities. Requirements

Graduate of an accredited Certificate, Diploma, Associate, or Bachelor’s Degree nursing program
Current, unrestricted Registered Nurse license in the state of Massachusetts
At least one year of nursing experience in a clinical care setting
Strong assessment, communication, organization, and care-coordination skills
Ability to work independently while collaborating effectively with an interdisciplinary team
Willingness and ability to travel approximately 50% within the Worcester-area coverage territory
Dependable vehicle, valid driver’s license, and current automobile insurance in accordance with state requirements
Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.

Nice To Haves

Behavioral health, psychiatric nursing, home health, or hospice experience preferred
Associate or Bachelor’s Degree in Nursing preferred
Homecare HomeBase experience preferred

Responsibilities

Manage an assigned caseload and coordinate individualized care for patients with behavioral health and coexisting medical needs.
Conduct comprehensive assessments to identify each patient’s physical, behavioral, psychosocial, environmental, and safety needs.
Develop, implement, coordinate, and evaluate individualized plans of care.
Complete initial evaluations and reassess each patient’s nursing needs during subsequent visits.
Provide skilled behavioral health nursing visits based on the patient’s plan of care.
Monitor changes in the patient’s mental, emotional, and physical condition.
Educate patients and caregivers about diagnoses, medications, symptom management, coping strategies, and available community resources.
Support medication adherence and identify potential medication concerns or side effects.
Recognize changes in condition, safety risks, or potential crises and communicate concerns promptly to the appropriate clinical team members.
Collaborate with physicians, therapists, caregivers, facilities, and other members of the interdisciplinary care team.
Revise the plan of care when patient needs or circumstances change.
Provide patients and families with the education, resources, and support needed to promote stability and independence at home.
Supervise and document care provided by behavioral health aides and LPNs, when applicable.
Maintain timely, accurate, and complete clinical documentation in accordance with agency policies and regulatory requirements.
