About The Position

The Behavioral Health Provider is a clinical staff member supporting our model of patient-centered, team-based care that integrates behavioral health with primary care, leading to improved patient health outcomes and overall satisfaction. This position is focused on substance use disorders and co-occurring disorders treatment. The Behavioral Health Provider will work closely with Primary Care Providers and the SUD Care Coordinators in the Medication-Assisted Treatment (MAT) program to provide behavioral health support. While the position will primarily be located at one (1) site for work, the Behavioral Health Provider will receive referrals from across PrimeCare.

Requirements

  • Master’s degree in Social Work, Counseling, Psychology. LCSW or LCPC strongly preferred.
  • Possess all licenses, registrations, certifications, and accreditations required by law to render health care services in the State of Illinois.
  • Minimum of three (3) years of clinical experience as a behavioral health provider in a health care setting required.
  • Required to work a minimum of one (1) evening per week.

Nice To Haves

  • Highly skilled in providing behavioral health services, with prior experience working in the substance use disorder field strongly preferred.
  • Interest in providing services to individuals, families, and couples.
  • Experience with motivational interviewing and harm reduction approaches.
  • Knowledge and/or experience of substance use disorders, co-occurring disorders, trauma treatment, and working within a multi-disciplinary team.
  • Preference given to individuals who are bilingual in English/Spanish.

Responsibilities

  • Provide substance use disorder/co-occurring disorder treatment to patients in an outpatient setting,
  • Complete assessments and provide interventions/therapy to patients referred for substance use treatment, including patients receiving services through the MAT program.
  • Conduct risk assessments and support patients in crisis, as indicated.
  • Coordinate care with team by responding in a timely manner to communication via phone, email, or athenaText (EMR).
  • Complete clinical notes and other necessary paperwork in a timely manner to comply with all administrative regulations, including completion of all visit notes within 72 business hours of visit.
  • Educate the care team on basic strategies for supporting patients experiencing substance use/co-occurring disorders.
  • May participate in street outreach, as directed by manager.
