About The Position

This position is responsible for conceptualizing, recommending, developing, and implementing Behavioral Health Outcomes Based Care efforts in multiple CCO regions. Additionally, this position guides implementation and maintenance of plans and procedures through data informed technical assistance, coaching, learning events, and training to Behavioral Health plan employees and network providers in areas related to Outcomes Based Care, Quality Improvement, and behavioral health quality metrics across multiple CareOregon regions. This position leads projects and facilitates collaborative meetings, providing support to network leadership and care teams related to outcomes-based care in order to improve Quintuple Aim outcomes in Behavioral Health services. This requires a high degree of collaboration and influence with multiple internal stakeholders as well as external agencies and coordination with external clinical and operational leadership. NOTE: This position is open to candidates residing in Oregon and SW WA. Please submit a cover letter along with resume. Requirements

  • Master’s degree in social work , counseling, public health or related field
  • Valid LCSW, LPC, or LMFT credential - OR - valid QMHP credential AND 3 years’ experience working in a behavioral health direct care setting
  • Minimum 5 years’ related experience (e.g., Behavioral Health Case Manager, Care Coordinator, Quality Improvement Specialist, etc.) that includes the following:
  • 2 years’ experience in behavioral health outcomes-based care
  • 2 years’ experience designing and delivering training and/or technical assistance to professional-level audiences
  • Experience in the Medicaid environment

Responsibilities

  • Research approaches to outcomes-based care and market practices; assist the development and implementation of an outcomes-based care project plan and resulting outcomes-based care projects for multiple CCO regions.
  • Design and implement a variety of projects relating to implementing outcomes-based care in behavioral health networks, including documentation of project goals, monitoring progress and quality of outcomes, and ensuring delivery of results.
  • Monitor and regularly communicate project status to ensure progress toward completion and within agreed upon timelines, budget, etc.
  • Serve as primary liaison with web-based outcomes application vendor(s) for outcomes initiatives and facilitate and/or coordinate analysis of clinical measurement tools.
  • Analyze and effectively communicate complex data for internal and external parties (e.g., narratives, charts, tables, graphs, etc.) to convey meaning, significance, and actionable information.
  • Provide independent and collaborative on-going support to clinical and operational leadership teams and staff of in-network providers in the areas of Behavioral Health outcomes-based care.
  • Develop and deliver support of local outcomes-based care resources to enhance service delivery in multiple CCO regions through a train-the-trainer program.
  • Support network providers through outcomes-based care projects; train clinic leadership teams to provide these services to their staff.
  • Assist providers in planning and reporting to receive payment for outcomes-based care implementation and reporting.
  • Collaborate with internal and external partners to help ensure effective coordination of development and planning efforts between multiple external community agencies and internal departments.
  • Participate in creating and implementing evaluation tools for key process and outcome measures.
  • Apply appropriate improvement tools and technology essential to meeting project goals and communicate with other departments as needed.
  • May provide in-person or onsite support to Network teams as needed and budget permits.
