About The Position

This role involves providing direct patient care in a behavioral health setting, focusing on assessments, care plan development, medication administration, and patient education. The nurse will manage a caseload, coordinate with the clinical team, and ensure continuity of care through effective communication with physicians and other healthcare professionals. A key aspect of the role includes supervising HHAs and LPNs, completing accurate documentation (OASIS, visit notes, orders), and maintaining compliance with all relevant policies, regulations, and HIPAA standards.

Requirements

• Active RN license
• Graduate of an accredited nursing program (BSN preferred)
• Home health or related behavioral health clinical experience
• Strong communication, organization, and critical thinking skills
• Current CPR certification
• Valid driver’s license and reliable transportation

Responsibilities

• Provide direct patient care, including assessments and care plan development
• Manage a caseload and coordinate care across the clinical team
• Administer medications and treatments per physician orders
• Monitor patient progress and adjust care plans as needed
• Educate patients and caregivers on care and self-management
• Communicate with physicians and care teams to ensure continuity of care

• Ensure compliance with policies, regulations, and HIPAA standards
