About The Position

This role involves providing direct patient care in a home health or related behavioral health setting. The nurse will be responsible for assessments, care plan development, medication administration, patient monitoring, and education. Collaboration with physicians and care teams is essential for ensuring continuity of care. The position also includes supervisory duties for HHAs and LPNs, and requires accurate and timely documentation, including OASIS and visit notes. Compliance with all relevant policies, regulations, and HIPAA standards is mandatory.

Requirements

• Active RN license
• Graduate of an accredited nursing program (BSN preferred)
• Home health or related behavioral health clinical experience
• Strong communication, organization, and critical thinking skills
• Current CPR certification
• Valid driver’s license and reliable transportation

Responsibilities

• Provide direct patient care, including assessments and care plan development
• Manage a caseload and coordinate care across the clinical team
• Administer medications and treatments per physician orders
• Monitor patient progress and adjust care plans as needed
• Educate patients and caregivers on care and self-management
• Communicate with physicians and care teams to ensure continuity of care
• Supervise and support HHAs and LPNs
• Complete accurate and timely documentation (OASIS, visit notes, orders)
• Ensure compliance with policies, regulations, and HIPAA standards
