Care at Home is a collaborative, population health–focused care delivery model serving Dual Eligible Special Needs Plan (DSNP) members—individuals with significant medical, behavioral, and social complexity, including serious mental illness (SMI) and substance use disorders (SUD). In this role, you will partner with an interdisciplinary care team to deliver high-quality, evidence-based behavioral health care. As a Behavioral Health APC (PMHNP), you will play a critical role within an interdisciplinary care team—bringing psychiatric expertise into a home-based and population health model of care. You will support both direct patient care and consultative care. If you have Oklahoma licensure, you will have the flexibility to work remotely as you take on some tough challenges.
