About The Position

Progressive Community Health Centers is seeking an experienced professional to serve as a Behavioral Health Navigator at our Lisbon Avenue Health Centers. This role involves implementing care plans designed by social workers to address patients' identified needs, resources, and goals. Key responsibilities include educating patients and their families about available options, prioritizing cases based on urgency, and effectively utilizing community resources. The position promotes patient self-management and independence by assisting with essential services such as transportation and housing. Additionally, the role requires maintaining accurate documentation, participating in departmental meetings, and fostering collaborative teamwork while adhering to organizational policies and ethical standards. The candidate must possess strong communication skills, cultural competency, and the ability to work independently and as part of a team. Knowledge of federal and state regulations, particularly regarding patient confidentiality and HIPAA, is essential. The schedule is Monday - Friday, 8 a.m. - 5 p.m., with no late nights, holidays, or weekends required.

Requirements

  • A bachelor's degree in social work, human services, psychology, public health, criminal justice, sociology, anthropology, or related field.
  • Experience preferred within Milwaukee County community resource settings.
  • Mandatory vaccination for all employees, employed providers and volunteers.
  • Proof of vaccination must be submitted prior to orientation.
  • Strong communication skills.
  • Cultural competency.
  • Ability to work independently and as part of a team.
  • Knowledge of federal and state regulations, particularly regarding patient confidentiality and HIPAA.

Responsibilities

  • Providing coordinated, patient-centered support that ensures timely, effective, and seamless delivery of care.
  • Identifies and effectively utilizes community resources to meet the needs of patients/families.
  • Participates in departmental and organizational committees as applicable.
  • Promotes collaborative teamwork.
  • Implements the plan developed by the Social Worker that addresses the patient's identified needs, issues, resources, and care goals; determining the choices available to individual patients; and educating the patient/family on available options; prioritizing patients according to intensity, need, and required follow-up.
  • Promotes patient self-management and empowers patients and families to achieve maximum wellness and independence.
  • Assists patients with needed items, including transportation, scheduling, housing, food sources, etc.
  • Maintains professional growth and development.
  • Maintains appropriate professional boundaries.
  • Interacts in a manner that is professional, respectful, positive, helpful, and promotes trust.
  • Acts in a manner consistent with the Code of Conduct.
  • Maintains accurate and timely documentation.

  • Provides input into work processes and policy revisions.
  • Maintains a working knowledge of and adheres to applicable federal/state regulations, including, but not limited to, laws related to patient confidentiality, release of information, and HIPAA.
