About The Position

The Behavioral Health Navigator provides outreach, engagement, care coordination, and navigation services for individuals participating in or eligible for the Hep C program, including those diagnosed with Hepatitis C and those at risk. This role supports clients in accessing and engaging with comprehensive services including medical care, mental health, psychiatry, substance use disorder (SUD) treatment, and social services. The Navigator offers advocacy and follow-up to promote continuity of care and improved health outcomes, navigating clients through complex systems of care, addressing barriers to access, and ensuring successful connection to needed services.

Requirements

  • Requires a high school diploma, GED, or equivalent required.
  • Understanding of recovery principles and/or behavioral health systems.
  • Two years of experience performing related duties such as outreach, peer support, case management, or care coordination.
  • Knowledge of or willingness to learn about Hepatitis C, including prevention, treatment, and engagement strategies.
  • Experience working with diverse populations, including individuals impacted by substance use disorder and intergenerational trauma, with familiarity in trauma-informed care principles
  • Ability to engage and motivate clients.
  • Excellent written and oral communication skills.
  • Basic understanding of common medical conditions and terminology.
  • Demonstrated ability and commitment to working collaboratively as part of a multidisciplinary team.
  • Ability to work in both indoor and outdoor environments and effectively manage priorities.
  • Willingness and ability to work a flexible schedule.
  • Must pass a pre-employment or for cause drug tests.
  • Must pass criminal background and DMV checks

Nice To Haves

  • Experience working within the Native American/Alaska Native community is a plus.
  • Experience working with individuals affected by substance use, homelessness, or chronic health conditions such as Hepatitis C preferred.
  • High degree of cultural sensitivity and willingness to learn.

Responsibilities

  • Conduct outreach and engagement with individuals who have Hepatitis C or are at risk, including those not yet connected to care.
  • Serve as a point of contact to help clients navigate and access internal and external services, including primary care, Hep C treatment, mental health, psychiatry, SUD treatment, and social services.
  • Provide client-centered support, mentoring, and encouragement to promote engagement in care and recovery-oriented services.
  • Coordinate referrals and actively follow up to ensure successful linkage to services and continuity of care.
  • Escort clients to appointments (medical, behavioral health, or social services) when appropriate.
  • Collaborate with interdisciplinary teams including medical providers, behavioral health staff, case managers, and housing/support service staff.
  • Assist clients in overcoming barriers to care, including transportation, scheduling, and system navigation challenges.
  • Support health education efforts, including education related to Hepatitis C prevention and treatment.
  • Document all outreach, engagement, and care coordination activities in a timely and accurate manner.
