About The Position

The Behavioral Health Manager, Nurse Practitioner-Integrated Care Delivery (NP-ICD) is a senior clinical leader responsible for delivering and scaling high-quality, integrated behavioral health services for complex Medicare members. The role blends direct clinical care with multidisciplinary leadership and program development to ensure safe, evidence based, and coordinated care across the Embrace model. Operating in a hybrid capacity, the role balances approximately 70% direct member care with 30% strategic leadership and program oversight. The Behavioral Health Manager, NP-ICD leads and develops the behavioral health team, partners closely with primary care and multidisciplinary stakeholders, and drives improved access, quality, and outcomes through a collaborative care model that supports member stability and prevents avoidable events across the continuum of care.

Requirements

  • Graduate or Advanced Degree required from an accredited school of nursing
  • California RN, NP, and Furnishing licensure required.
  • Certification in area of specialty required.
  • 5+ years of recent NP experience required.
  • Demonstrated leadership and management skills.
  • Strong strategic thinking skills to facilitate the development and delivery of new clinical products and services to seniors and the community.
  • Strong interpersonal skills, excellent written and oral communication skills; commitment to detail.

Nice To Haves

  • 3+ years supervisory experience preferred.
  • Demonstrated experience in working with both Medicaid and Medicare preferred.

Responsibilities

  • Provide short term, evidence based behavioral health treatment to members, including assessment, diagnosis, treatment planning, individual and group interventions, and discharge/aftercare planning.
  • Manage members with mild to severe mental illness, applying risk stratification and care intensity appropriate to clinical need.
  • Develop and oversee individualized, whole person care plans aligned with evidence based guidelines, clinical best practices, and community standards.
  • Provide behavioral health education and coaching to members, caregivers, and care teams, including psychosocial interventions and pharmacologic considerations.
  • Maintain a limited direct clinical panel while balancing programmatic and leadership responsibilities to ensure ongoing clinical relevance and member impact.
  • Partner closely with primary care providers, psychiatry, case management, caregivers, and community resources to support member safety, stability, and independence in the community.
  • Lead multidisciplinary case conferences and psychiatric rounds, ensuring alignment on treatment plans, escalation pathways, and follow up.
  • Support safe and effective behavioral health medication management in collaboration with PCPs, psychiatric consultants, and pharmacy resources, including monitoring, optimization, and timely follow up.
  • Design, operationalize, and continuously improve behavioral health pathways, triage standards, and escalation processes for complex cases.
  • Establish consistent clinical standards, throughput expectations, and outcome measures across the behavioral health program.
  • Identify and implement process improvements to enhance access, quality, efficiency, and member experience within the integrated care model.
  • Develop, monitor, and refine success metrics, using data to drive improvement and scalability.
  • Provide clinical and operational oversight for an integrated behavioral health team, which may include LCSWs, Community Health Workers (CHWs), and Behavioral Health Pharmacist.
  • Recruit, onboard, train, mentor, and develop behavioral health staff to support workforce stability and clinical excellence.
  • Manage staff performance, supervision, scheduling, and access standards; proactively scale internal capacity to reduce reliance on fragmented external vendor models.
  • Ensure delivery of care aligns with ethical standards, regulatory requirements, and organizational quality expectations.
  • Collaborate with senior leadership to resolve operational and performance challenges and to design new programs and care models.
  • Partner across departments to integrate behavioral health seamlessly with primary care, case management, and pod based workflows.
  • Build strong relationships with internal and external stakeholders to support membership growth, quality outcomes, and long term program sustainability.
  • Ensure strict adherence to HIPAA, regulatory, compliance, and documentation standards, completing all clinical and operational documentation accurately and timely.
  • Support professional growth through continued education, engagement in professional organizations, and maintenance of clinical best practices.
  • Actively support and advance SCAN’s vision, mission, and strategic goals.
  • Contribute to team and organizational success by identifying opportunities to achieve shared goals and targets.
  • Supervises/Managers Others (i.e. hires, performance reviews, corrective action, etc.)
