About The Position

The Behavioral Health LPN Coordinator is an experienced and skilled member of the Behavioral Health team responsible for coordinating behavioral health services, supporting patient engagement, and facilitating continuity of care throughout the patient journey. Working closely with psychiatric providers, care teams, and community resources, the Behavioral Health LPN Coordinator helps ensure patients receive timely, high-quality, integrated behavioral health services. This role serves as a key liaison between patients and the interdisciplinary care team by assisting with care coordination, patient education, documentation, quality improvement activities, and program support to promote positive health outcomes and access to care.

Requirements

  • Active unrestricted LPN license in Pennsylvania required.
  • Minimum of two years of nursing experience required.
  • Experience working with electronic health records required.
  • Strong organizational, communication, and interpersonal skills.
  • Ability to work collaboratively within a multidisciplinary team.

Nice To Haves

  • Behavioral health, care coordination, or case management experience preferred.
  • Knowledge of behavioral health conditions, treatment modalities, and community resources.
  • Bilingual in English and Spanish preferred.

Responsibilities

  • Coordinates behavioral health services and patient care activities, including referrals, new patient appointments, follow-up visits, telepsychiatry services, and communication among providers and care team members.
  • Assist patients in navigating behavioral health services by providing education, support, resource coordination, and follow-up to promote engagement and continuity of care.
  • Responds to patient inquiries, gathers pertinent clinical information, assists with medication refill processes within scope of practice, and communicates patient concerns to appropriate providers.
  • Collaborates with providers, behavioral health staff, and community resources to support coordinated, patient-centered care.
  • Room and prepare patients for scheduled appointments.
  • Prepare and administer medications and injections as directed by providers.
  • Maintains accurate and timely documentation in the electronic health record and assists with data collection, chart audits, reporting requirements, and regulatory compliance activities.
  • Participates in quality improvement activities and supports departmental and organizational initiatives to improve patient outcomes and access to care.
  • Maintain patient confidentiality and comply with all CHDC policies, procedures, and applicable regulatory requirements.
  • Demonstrates professionalism, reliability, flexibility, and commitment to CHDC's mission, vision, and values. Performs other duties as assigned.
