About The Position

Center for Human Development (CHD) is seeking Behavioral Health Licensed Practical Nurses (LPN) to join the team at Heron House, an Enhanced Medical Group Living Environment (EMGLE) program. This 18-bed facility in East Longmeadow, MA, offers a low LPN to client ratio, allowing for quality care and support. Working under the supervision of a charge nurse or RN, you will collaborate with a support team of LPNs, CNAs, and Direct Care staff in a modern EMGLE facility. The EMGLE program provides skilled healthcare and supportive services, including 24/7 nursing and personal care assistance, to individuals with mental illness and complex medical needs. These services aim to support daily needs, manage chronic medical conditions, terminal illnesses, and/or disabilities impacted by significant mental illness. The program requires licensed and unlicensed professionals to address chronic complex medical and behavioral needs, aiming for improvement or stabilization, prevention of worsening conditions, and avoidance of hospitalization or nursing facility admissions.

Requirements

  • Have a license in practical nursing by The Board of Registration in Massachusetts.
  • Meeting physical demands of the role. IE: Ability to lift 45 lbs. or more, stocking supplies, extended periods of walking and/or standing.
  • Participate and successfully complete assigned training requirements in compliance with CHD and the Massachusetts Department of Mental Health.

Nice To Haves

  • One-year experience as a LPN, preferred.
  • One-year experience in a Behavioral Health setting, preferred.
  • One-year experience working with the elderly, people with disabilities, behavioral challenges and cognitive disabilities, preferred.
  • Valid driver’s license.
  • Strong written skills and experience with Electronic Health Record Management is a plus.

Responsibilities

  • Medication administration and management.
  • Assist in medication audit processes.
  • Attend participants care plan meetings/resource meetings.
  • Provide direct care as needed.
  • Providing personal hygiene care by assisting with travel to the bathroom; helping with showers and baths.
  • Effectively understand details about a patient’s health and provide charge nurse or RN with clear, precise information and observations.
  • Provides activities of daily living by assisting with serving meals, feeding Persons as necessary, ambulating.
  • Provides adjunct care by administering nonsterile dressings, surgical preps, ice packs, and heat treatments.
  • Maintains the stability of Persons served by checking vital signs and weight, recording intake and output information.
  • On call monthly rotation (stipend provided, plus hours wage if called in to pass meds).
