About The Position

Coordinates and assists in the provision of Behavioral Health resources and services to clients. This role involves assisting families in determining needed resources and services through a screening and triage process, answering questions and concerns, and providing service referrals and scheduling. The coordinator will gather referral and pre-admission information, complete data entry, and identify emergent or complex cases for clinical review. Collaboration with interdisciplinary teams is essential for efficient resource and scheduling services. The position requires utilizing Electronic Medical Record and Scheduling software to assess service demand and capacity, and maintaining strict confidentiality of all clinical and administrative information.

Requirements

  • Bachelor’s Degree in psychology, social work or related field, required.
  • Strong skills in customer service, process improvement, data entry, and advanced computer skills, required.
  • Behavioral health or customer service experience, required.
  • Bend/twist, Climb stairs/ladder, Flexing/extending of neck, Lifting / Carrying: 0-10 lbs, Pushing / Pulling: 0-25 lbs, Reaching above shoulder, Squat/kneel (Occasionally)
  • Standing, Walking (Frequently)
  • Audible speech, Color vision, Computer skills, Decision Making, Depth perception, Hand use: grasping, gripping, turning, Hearing acuity, Interpreting Data, Peripheral vision, Problem solving, Repetitive hand/arm use, Seeing – Far/near, Sitting (Continuously)

  • Proficiency with EPIC and the NCHBH system, preferred.

Responsibilities

  • Assists families in determining needed resources and services through a screening and triage process.

  • Identifies emergent or complex cases and seeks clinical review prior to finalizing the referral.
  • Collaborates with interdisciplinary teams to ensure efficient resource and scheduling services.
  • Utilize Electronic Medical Record and Scheduling software to assess the demand for services and the ability of current service capacities to meet the community’s needs.
  • Maintains confidentiality of all clinical and administrative information consistent with agency policy.
