About The Position

The Registered Nurse (RN) – Medicaid Health Home is responsible for delivering integrated, whole-person, person-centered care coordination services to Medicaid beneficiaries with Serious Mental Illness (SMI), Serious Emotional Disturbance (SED), Substance Use Disorders (SUD), and/or chronic medical conditions. The RN supports compliance with Medicaid Health Home core service requirements, promotes quality outcomes, reduces avoidable hospitalizations and emergency department utilization, and ensures coordination across physical health, behavioral health, and social service systems.

Requirements

  • Graduation from an accredited school of Nursing
  • Licensed as a registered nurse (RN) in the State of Michigan
  • Two (2) years of clinical nursing experience
  • Experience working with individuals with SMI, SUD, and/or chronic medical conditions
  • Knowledge of Medicaid regulations, managed care processes, and care coordination models.
  • Must meet third party billing requirements.
  • Must have a valid Michigan driver’s license, current automobile insurance, and the use of a vehicle during work hours, including for the transport of clients.
  • Ability to visually assess details at both near and far distances, as well as to perceive depth and spatial relationships.
  • Ability to receive and respond to verbal communication.
  • Ability to lift up to 35lbs on an occasional basis.
  • Ability to regularly stand for extended periods of time.
  • Ability to walk at a minimum of 35% up to a maximum of 65%.
  • Ability to occasionally stoop, bend, or reach.
  • Ability to perform tasks requiring manual dexterity, including the use of hands and fingers for handling, grasping, typing, writing, or operating equipment.
