Requirements

  • Masters Degree in Social Work with 2 years post graduate experience in health care setting
  • Minimum of LSWA, LGSW or LCSW. LCSW-C preferred

Nice To Haves

  • Fluency in Spanish preferred

Responsibilities

  • Provides remote behavioral health assessments, crisis intervention, discharge planning and resource linkage for Emergency Department patients of all ages during evening, weekend, and holiday coverage hours.
  • Conducts suicide and homicide risk assessments, rapid clinical evaluations, safety planning, disposition planning and coordination of psychiatric and community-based services.
  • Collaborates with Emergency Department staff, psychiatry, nursing, case management and outside providers to ensure patients receive appropriate behavioral health care and follow-up services.
  • Conduct timely bio-psychosocial evaluations and crisis assessments for ED patients through a virtual platform.
  • Make rapid clinical assessments and decisions to ensure patients are linked to the appropriate level of care.
  • Review urinalysis and assess how substance use may contribute to psychiatric symptoms and mental health concerns.
  • Complete suicide/homicide risk assessments, safety planning, and crisis intervention.
  • Determine appropriate level of care, including inpatient, residential, outpatient, detox, crisis stabilization, or discharge planning.
  • Coordinate psychiatric placements and community resource referrals.
  • Collaborate with ED staff, psychiatry, nursing, case management, and outside providers.
  • Provide supportive counseling, de-escalation, and patient/family support.
  • Maintain timely, accurate documentation and compliance with hospital and regulatory standards.
  • Provide on-call coverage and respond to consults within departmental expectations.
