About The Position

Hampton-Newport News Community Services Board (H-NNCSB) is seeking a Behavioral Health Docket Case Manager to provide case management and behavioral health support to individuals with serious mental illness who are involved with the criminal justice system. H-NNCSB partners with the Hampton and Newport News General District Courts through separate Behavioral Health Dockets (BHDs). Behavioral Health Dockets provide a collaborative, non-adversarial approach that brings together the court system, behavioral health providers, and other community partners to support individuals with serious mental illness who have become involved in the criminal justice system. Through ongoing support, supervision, treatment coordination, and advocacy, the Behavioral Health Docket team helps participants engage in services, address barriers, and reduce further involvement with the criminal justice system.

Requirements

  • Bachelor's degree in Human Services, such as Social Work, Psychology, or Counseling.
  • At least one (1) year of experience providing case management services.
  • Must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.

Responsibilities

  • Serve as a liaison between participants, courts, criminal justice agencies, families, and service providers.
  • Assess participant needs and develop, monitor, and update treatment and service plans.
  • Provide case management, advocacy, counseling, crisis intervention, and daily living support.
  • Coordinate referrals and services related to behavioral health, substance use, housing, employment, medical care, and community resources.
  • Prepare reports and documentation for Behavioral Health Docket hearings and attend or testify at court proceedings as needed.
  • Facilitate life skills groups and participate in case review and treatment planning meetings.
  • Assist participants with obtaining basic resources and maintaining community stability.
  • Coordinate risk assessments, crisis plans, discharge planning, and continuity of care.
  • Maintain housing and community resource information.
  • Conduct on-site urine drug screenings for participants with substance use disorders.
  • Maintain timely and accurate case management records and documentation.
