We are seeking a full-time, 40-hour Integrated Behavioral Health Counselor to support our Primary Care of Rochester practice located at 150 Marketplace Boulevard, Unit 9, Rochester, NH. This position operates within the practice's hours of operation of Monday through Friday, 8:00 AM – 5:00 PM. Schedules may be staggered and offer flexibility based on provider availability and the needs of the practice. The role will be fully onsite for the first six months of employment and will transition to a hybrid schedule of four days onsite and one day remote following successful onboarding and integration into the practice. In this role, the Integrated Behavioral Health Counselor will primarily support patients of the Primary Care of Rochester practice while also providing virtual coverage to at least one additional primary care site as needed. Working as part of an integrated care team, the counselor will collaborate closely with primary care providers, nurses, care managers, and other members of the healthcare team to provide short-term, solution-focused behavioral health services. Responsibilities include conducting behavioral health assessments, developing treatment plans, delivering brief interventions, and participating in warm handoffs from primary care providers. The counselor will provide both in-person and virtual appointments and will work independently while also receiving support from a network of licensed behavioral health clinicians across the organization. As part of our Integrated Behavioral Health Services team, you will work closely with primary care providers and interdisciplinary team members to deliver high-quality behavioral healthcare to patients across the lifespan.
