About The Position

The Behavioral Health Coordinator serves as the Behavioral Health Program Coordinator, overseeing quality and regulatory compliance for behavioral health patients. This role involves policy development, file auditing, addressing consumer complaints, and staff training. The coordinator also provides care coordination, including referrals and discharge planning, under provider guidance. They connect patients with mental health and social services, arrange consults, and educate families and stakeholders. Additionally, the role may involve functioning as a sitter for mental health patients and offering inpatient mental health resources. The coordinator works with a multidisciplinary team to address social determinants of health and supports patients facing issues like homelessness, substance abuse, and mental health challenges. Within the Emergency Department, they assist with EKG team troubleshooting, conduct chart audits, and support operational and clinical workflows.

Responsibilities

  • Serve as the Behavioral Health Program Coordinator.
  • Coordinate with leadership to ensure quality and regulatory compliance for behavioral health patients and the holding area.
  • Assist with policy development to meet regulatory requirements.
  • Participate in interviewing and making hiring recommendations.
  • Report care concerns or non-compliance to leadership.
  • Function as a sitter for mental health patients when needed.
  • Offer inpatient mental health resources.
  • Ensure behavioral health services comply with regulatory requirements.
  • Develop policies, audit files, and address consumer complaints.
  • Inform and train staff on new policies and procedures.
  • Provide care coordination, including referrals and discharge planning, under provider guidance.
  • Refer consumers to mental health or social services in the emergency department, inpatient, and 2East units.
  • Arrange consults and provide education to families, providers, and community stakeholders.
  • Advocate for patients and connect them to appropriate resources.
  • Assist with coordinating patient care, such as transportation, medical appointments, and follow-up care.
  • Work with a multidisciplinary team to support patients and address social determinants of health.
  • Link patients with resources for housing, transportation, and healthcare.
  • Work with patients facing homelessness, substance abuse, mental health issues, and complex chronic medical conditions.
  • Serve as a resource within the Emergency Department, assisting with EKG team troubleshooting, conducting chart audits, and supporting operational and clinical workflows.
  • Gather information on patients' social, psychological, environmental, and financial situations.
  • Identify factors affecting medical care and discharge plans.
  • Communicate critical information to the care team.
  • Ensure an appropriate environment and develop activities for behavioral health patients.
  • Coordinate telepsych and teletherapy consults for Emergency Department patients.
  • Coordinate inpatient psychiatric consults.
  • Initiate or assist in arranging intervention and referrals for treatment upon discharge.
  • Provide monthly stats to NCSTeP.
  • Assist with petitions for involuntary commitment.
  • Facilitate patients' access to services for better health outcomes.
  • Facilitate coordination/development of appropriate safety plans for discharge.
  • Communicate documentation and treatment plans with the referring provider.
  • Provide support for difficult discharge placements.
  • Receive referrals for post-acute care facilities and interview families for appropriate placement.
  • Ensure appropriate discharge documentation and coordinate transportation.
  • Document accurately and thoroughly.
  • Participate in hospital-based committees.
  • Represent EWCH behavioral health services in community/NC State-based committees.
  • Work with Business Development and Marketing to develop an outreach plan.
