The Behavioral Health Consultant (BHC) specializes in brief, evidence-based, solution-focused assessments and interventions geared toward functional improvement. BHCs are generalists, providing high-volume services from a population-based lens that are accessible, team-based, and a routine part of primary care. They will Identify, triage, and co-manage patients with medical and behavioral health problems within the primary care setting. They will provide skill training through psychoeducation and patient education strategies and will develop specific behavioral change plans for patients and behavioral health protocols for target populations. They will support both the primary care providers and the patients by providing quality clinical mental health consultations to providers and patients as requested. The Behavioral Health Consultant is a member of the Integrated Clinical Care team providing patient-centered care.
