This position will be joining Bloom’s Behavioral Health team, reporting to the Director of Specialty Services, with a focus on caring for Bloom’s geriatric patients with mental health challenges and/or dementia as well as young disabled patients with psychiatric needs.

Requirements

  • A bachelor's degree in psychology, counseling, social work, nursing, or a related field required.
  • Master's degree or higher in clinical psychology, counseling psychology, social work, or a related field.
  • LCSW/LSW, LPC, or PSY
  • Strong clinical skills, including the ability to assess mental health needs, develop treatment plans, and provide evidence-based interventions.
  • Knowledgeable about a range of therapeutic approaches, such as cognitive-behavioral therapy, dialectical behavior therapy, motivational interviewing, and solution-focused therapy.
  • Excellent communication skills are essential for building rapport with clients, collaborating with other members of the healthcare team, and providing education and support to patients and their families.
  • An understanding of and respect for cultural differences and how they may impact mental health and healthcare seeking behavior.
  • Skilled at problem-solving and be able to help clients identify and address challenges in their lives.
  • A strong sense of empathy and compassion is essential for working effectively with individuals who may be experiencing mental health challenges.
  • Must adhere to ethical guidelines and professional standards of conduct in their practice. This includes maintaining patient confidentiality, respecting boundaries, and seeking supervision or consultation when needed.
  • Ongoing professional development and continuing education are important for staying current with best practices, emerging research, and advancements in treatment modalities.

Responsibilities

  • Mental health assessments, including administration of validated scales e.g. PHQ-9, GAD-7
  • Provide lifestyle coaching to address issues such as anxiety, depression, sleep disturbances, and loss of function
  • Use multiple modalities to engage with clients like motivational interviewing, problem solving therapy, behavioral activation, and provide grief support
  • Coach caregivers and family members to support and advocate for patients, especially for vulnerable patients with issues such as dementia
  • Monitor symptom severity, treatment adherence and side effects
  • Track client progress through the use of a registry
  • Report results to primary medical provider and/or the consulting psychiatric consultant
  • Identify social service needs and refer to Bloom’s medical social work where appropriate
  • Participation in weekly caseload review with psychiatric consultant
  • Ongoing collaboration and coordination with treating providers
  • Relapse prevention planning and preparation for discharge from active treatment
  • Care management to be delivered in client’s homes or assisted living communities, but some virtual visits may be warranted
