About The Position

The position will provide behavioral health assessments, brief evidenced-based interventions, clinical/consultative services and education using the Primary Care Behavioral Health (PCBH) model, for patients and their family in Jefferson ambulatory practices of varying specialties. The Behavioral Health Consultant (BHC) in this role will serve patients ranging in age from toddler through late adulthood. The BHC will also provide consultation with the Primary Care Provider (PCP) to enhance the understanding of the patient, provide decision support for treatment planning, and assist in the ongoing care and monitoring of patients' behavioral health care and biopsychosocial treatment plans. Ensures behavioral health services are delivered in a culturally appropriate fashion and is consistent with State laws and regulations, professional standards of care, agency guidelines, and professional ethical guidelines. This position will be supporting Women's Health specialty within Jefferson Health Family Medicine- Integrated Behavioral Health.

Requirements

  • Master’s Degree in Counseling relate field or social work from a School of Social Work accredited by the Council on Social Work Education or an equivalent degree as determined by the Council on Social Work Education Doctorate PhD/PsyD in Psychology at an accredited university or college.
  • 1 year in a behavioral health setting is required.
  • Knowledge of: Medical terminology and disease process; documentation best-practices
  • Skill in: Work with multidisciplinary teams, patients/families/significant others
  • Ability to: Establish an appropriate plan to safely discharge patients to the next level of care as well as understand and adhere to city, state, and federal regulatory requirements
  • LCSW - Licensed Clinical Social Worker - State of Pennsylvania or LPC - Licensed Professional Counselor - State of Pennsylvania or LP - Licensed Psychologist - State of Pennsylvania

Responsibilities

  • Integrates evidence-based practice, clinical expertise, patients, their family/designated care partner(s) preferences and cultural beliefs and values for delivery of optimal health care.
  • Partners with patients, their family/designated care partner(s) in identifying learning needs and priorities and provides education in ways that are useful, understandable and affirming.
  • Assist primary health care providers in recognizing, treating and managing patients with chronic emotional, psychosocial, substance use and/or physical health problems within the primary care setting.
  • The BHC will provide functional strengths-based assessments, psychoeducation, brief problem-solving cognitive intervention, medication adherence counseling, and disease self-management counseling among others.
  • Triaging and referring patients to specialty mental health care when appropriate.
