About The Position

The Behavioral Health Community Health Partner (BH CHP) manages a member panel to support care coordination and social care for high-complexity members with acute behavioral health conditions. Responsibilities: Assess/Intake Conducts assessments and screening instruments, including those for behavioral health, home safety, and social needs, while adhering to established protocols and maintaining regular check-ins with members. Collaborates with psychiatrists, RNs, Behavioral Health Specialists (BHS) and external providers as needed for case review, care planning, and to develop and maintain member care plans. Meets with members in the community (home, short-term nursing facility shelter, hospital) as needed, including as an extension of the care team for non-clinical needs Operations. Utilizes our care facilitation, electronic health record and scheduling platforms as needed to collect data, document member interactions, organize information, track tasks, and communicate with the team, members, and community resources. Requirements

  • 2+ years of progressive experience in care coordination, community health, health promotion, or a related field, working with individuals experiencing behavioral health conditions (e.g., substance use disorders or serious mental illness) in a case management or comparable role.
  • Medical Clearance (for Member-Facing Roles): You must complete Cityblock’s medical clearance requirements, which include, but may not be limited to, evidence of immunity to MMR, Hepatitis B, Varicella, and a TB screen, or have an approved medical or religious accommodation that precludes you from being vaccinated against these diseases.
