About The Position

The Behavioral Health Clinician is integrated into the primary care team, performs triage, and manages patients with behavioral health problems in collaboration with office-based resources. The Behavioral Health Clinician provides clinical mental health consultation to clinicians as requested and is also available to provide diagnostic mental health assessments, interventions, education, and referrals to escalate to higher acuity mental health services. This Clinician will also carry a small caseload of patients for ongoing therapy in the traditional outpatient setting.

Requirements

  • Able to communicate effectively (both verbally and in writing).
  • Positive interpersonal skills required.
  • Terminal degree in mental health related field: Master’s or Doctoral Degree in Social Work, Licensed Marriage & Family Therapy, Professional Counseling, Psychology, etc. required.
  • Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC).
  • Current certificate Basic Life Support is required for ambulatory privileges and additional certifications as required for Advanced Life Support skills for inpatient privileges.

Nice To Haves

  • 3+ years of experience in a clinical setting is strongly recommended.

Responsibilities

  • Provides consultation to and coordinate care of patients with primary care staff.
  • Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.
  • Identifies, refers, and advocates for patients needing specialty behavioral health services and other services as needed.
  • Assists in the detection of “at risk” behavioral health patients and the development of plans to prevent further deterioration.
  • Provides brief, solution-focused behavioral interventions with active development of behavioral change plan.
  • Teaches patients and families about care, prevention, and treatment enhancement techniques.
  • Attends and participates in meetings and quality improvement activities as required.
  • Assists primary health care providers in recognizing and treating mental disorders and psychosocial challenges through consultation, collaborative care appointments, as well as formal and informal teaching.
  • Assess the clinical status of patients referred by primary care providers through brief consultative contacts, management of referral work queue, etc.
  • Works with primary care team to efficiently and effectively treat and manage patients with chronic and new onset emotional and/or health problems.
  • Creates and oversee administration of clinical level processes/initiatives/pathways to manage behavioral health related quality measures such as screening for depression and anxiety, depression follow-up and remission, etc.
  • Provides care necessary as described in department policies and procedures.
  • Participates as a collaborative team member and maintains a current level of theory and practice in the appropriate field.
  • Keeps current with professional literature and trends in mental health.
  • Attends continuing educations trainings, workshops, seminars or conferences in order to keep current with evidence-based standards of practice and maintain licensure.
  • Attends clinical supervision meetings and staff meetings as scheduled and comes to meetings prepared.
  • Maintains appropriate records of service rendered to or on behalf of the client populations served.
  • Creates a positive milieu in the practice that fosters learning and development.
  • Other related duties as assigned.
