The Behavioral Health Clinician works in conjunction with members of the medical and behavioral health team to address the psychosocial aspects of both chronic and acute conditions. This includes applying behavioral health principles to support lifestyle changes, reduce health risks, and provide consultation in the treatment of mental health and psychosocial concerns. This role primarily serves individuals experiencing homelessness, as well as those impacted by substance use, mental health conditions, and histories of incarceration—often with complex, high-acuity needs. In a Clinical Care Coordinator capacity, the position also incorporates care coordination and team support, helping guide care planning, collaborating closely with interdisciplinary staff, and ensuring clients are connected to services such as psychiatric care, medication-assisted treatment, and housing resources. While maintaining a strong clinical focus, the role supports a more integrated, team-based approach to care for vulnerable populations.
